Get Support for Managing Complex Conditions with our Care Management Team

Our Care Management programs are available to help members with long-term health challenges. Experienced care managers and care coordinators will help manage your care and your relationships with health care providers. If you are going through a period of illness or managing a chronic health condition, we encourage you to take advantage of this special program to manage your health.

Health New England's Care Management team provides equitable, quality care and services to its culturally and linguistically diverse population, no matter the individual’s personal characteristics. Care Management's intent is to reduce health care inequities and disparities by implementing interventions for identified individuals who are likely to experience or are experiencing obstacles to health care services due to their race/ethnicity, language preference, gender identity, and/or sexual orientation. All of our Care Management Programs are offered to our members at no additional cost.

This program is available to help Medicare Advantage members plan a smooth transition after a hospital stay. We offer one-on-one support from a certified nurse at no additional cost. The nurse offers coaching that will help guide you before and during your transition to home or a rehabilitation facility.

Health New England offers eligible Medicare Advantage members a comprehensive in-home health visit with a medical professional at no additional cost. If you meet the criteria for eligibility, you will be contacted by our in-home health vendor to schedule a visit at your convenience in your home. The visit is nearly an hour of uninterrupted time with a licensed, board-certified medical provider who reviews your medical history and medications, and can answer your health questions.

We partner with Healthmap Solutions to deliver comprehensive care for members with chronic kidney disease (CKD) and end-stage renal disease (ESRD). Healthmap’s Kidney Health Management (KHM) program works with your doctor to identify and support your health needs, while helping you manage your chronic condition.

Members with opioid use disorder (OUD) can receive coverage of services to treat OUD through an Opioid Treatment Program. 

The Nutritional Grocery Store Program** is designed to engage and empower members who have at least two chronic conditions (diabetes, cardiovascular disease or congestive heart failure) to improve their health by focusing on nutrition. Health New England is partnering with Big Y® Supermarkets and their consulting registered dietitians to offer eligible members a grocery store tour (in-person or virtual) to help you develop a better understanding of healthy shopping and healthy eating. After completion of the grocery store tour with a registered dietitian, you will receive a $100 reward for health-related food purchases at Big Y.

The Nutritional Grocery Store Program benefits mentioned are a part of a special supplemental program for the chronically ill. Not all members qualify. This benefit does not apply to Compass PPO plans.
